This is a Teletype MHP 10 (1/14 HP), AC Motor Model #5SPM49DG6AX, manufactured by General Electric. The motor operates on 115 Volts, 60/50 Hz, 1800/1500 RPM Counter-Clockwise Rotation, 0.45/0.5 Amps, Single Phase. 

The motor has two 5/16" Diameter by 1" Length shafts, with one flatted, Class B Insl, is Thermally Protected, and rated continuous duty. This motor will need a capacitor.
